Registration Form & General Policies
Please fill out this registration form as it helps us build lesson schedule lessons, and assists with communication.
Fishers Guitar does not share your information with any third party.

If, after submitting this registration, you elect to engage Fishers Guitar as your music lesson provider, ​you acknowledge that you fully understand and will abide by, if applicable, the policy and terms as stated below.

Our Policies - PLEASE READ
STUDENTS

Students must attend all scheduled live and/or remote lessons, be on time, and bring all assigned materials, including music, music books, picks, capos, drumsticks, etc.

Students are required to practice daily for the amount of time determined between the student/parent and their instructor.
ATTENDANCE

Students may not elect to miss or deduct payment for missed lessons at anytime. In the event of illness, private lessons will either be made up or refunded at the instructor's discretion. In case of illness, please send an immediate email or text the evening before or the morning of the absence to: Tx 317-503-1390 Em: Fishersguitar@icloud.com.

In the event of a "no-show" - live or remote, the lesson time is charged the full rate and cannot be made up.

Private students are given one (1) unpaid vacation absences per school year. Please notify the instructor via email, at info@FishersGuitar.com, before you plan to be absent. (i.e., spring break, etc.)

Group classes cannot be made up unless a majority of the class is absent.

In the event the instructor must cancel any lesson (private, group or class), the lesson will either be made up or deducted from the lesson fee at the instructor's discretion. Live lessons may be substituted for remote.

If a student must withdraw from scheduled lessons, a four (4) weeks notice via email is necessary. Withdrawn lessons may not be picked up at a later date without prior approval from Fishers Guitar.
PAYMENT

Tuition is is due, in full, on the first day of every month.

There is a yearly registration/materials charge of $30.00 for all new students.
Tuition Payment & Terms

Normal lessons are payable monthly. Lesson payment is due in full on the first day of every month.

Students may not elect to deduct payment for missed lessons at anytime.

6 days after the invoiced tuition due date, a $10.00 late fee will be applied on late/unpaid accounts. A statement will be made available the prior month with all
fees due.

Checks, cash, major credit cards, digital payment services, are accepted. Auto debit may be arranged upon request. Online payment is available.

This website offers visitors various music lesson services and musical retail items. The prices Fishers Guitar charge for using our services and/or products are listed on the website and through the Marketing Services payment link via this website. We reserve the right to change our prices for products displayed at any time, and to correct pricing errors that may inadvertently occur. Additional information about pricing and sales tax is available on the payments page.
At the discretion of Fishers Guitar/Marketing Services, the fee for the goods and services and any other charges you may incur in connection with your use of the service, such as taxes and possible transaction fees and or late fees, will be charged either on a monthly basis to your payment method or at the point of purchase.
